Election Identification Birth Certificate (Search or Issued Certificate)

Death Certificates (Search or Issued Certificate)
$21.00 for 1st copy & $4.00 for each additional copy
obtained at the same time on the same individual

Fee rates are set by the Texas Board of Health and are not mandated by the
Texas Legislature. For any search where a record is not found, the search fee
is NOT refundable or transferable

IMPORTANT NOTICES
A BIRTH CERTIFICATE IS A VALUABLE LEGAL DOCUMENT.  DO NOT LOSE OWNERSHIP OF A BIRTH CERTIFICATE ISSUED BY A
STATE, LOCAL, OR COUNTY AGENCY.

If a school, sports league, medical facility or other organization desires proof of birth; it is essential that you present the birth certificate for viewing as proof of birth, age and/or relationship ONLY.  The requesting organization should NOT make a copy or keep a copy due to the amount
of private information.  Some government agencies such as the Social Security Administration or Passport Office require documents to be submitted with the application but later will return all such documentation to the owner. A birth certificate can only be issued 10 times within that individuals lifetime (Title 25, Texas Administrative Code).

PLEASE NOTE:  A hospital issued certificate of birth is not a legal document and generally is not accepted as proof of birth.

Mexican Matricula Consular and/or Mexican Electoral Cards, Foreign Birth Records, International Drivers License and Non-Citizen social security cards WILL NOT BE ACCEPTED AS IDENTIFICATION FOR BIRTH AND/OR DEATH RECORD REQUESTS

To request a copy of a Birth or Death Record:
Only a qualified applicant with the proper identification may obtain a copy of a vital record.  Vital records may be requested by coming to our offices at 808 Hwy 77-A South in Yoakum or by mail using the birth/death certificate application.  If you choose to complete the application online and mail it into us, please read the instructions carefully to ensure timely processing of your request.

We do not recommend mailing vital records for security purposes; if they must be mailed, we suggest using a trackable method such as certified mail, fedex, ups, etc.  For mailing options, please contact our office as additional fees will apply. Fee rates are set by the Texas Board of Health and are not mandated by the Texas Legislature. For any search where a record is not found, the fee(s) paid are not refundable.

AS THE LOCAL REGISTRAR’S OFFICE, WE HAVE THE RIGHT TO REFUSE AN APPLICANT AT ANY TIME.  Details of a qualified applicant and identification requirements are located within the links below.
