The City of Yoakum will no longer process Birth or Death Records. 

You may obtain Birth or Death Certificates from the State of Texas 

On-line at: www.texas.gov

is NOT refundable or transferable

IMPORTANT NOTICES
A BIRTH CERTIFICATE IS A VALUABLE LEGAL DOCUMENT.  DO NOT LOSE OWNERSHIP OF A BIRTH CERTIFICATE ISSUED BY A
STATE, LOCAL, OR COUNTY AGENCY.

If a school, sports league, medical facility or other organization desires proof of birth; it is essential that you present the birth certificate for viewing as proof of birth, age and/or relationship ONLY.  The requesting organization should NOT make a copy or keep a copy due to the amount
of private information.  Some government agencies such as the Social Security Administration or Passport Office require documents to be submitted with the application but later will return all such documentation to the owner. A birth certificate can only be issued 10 times within that individuals lifetime (Title 25, Texas Administrative Code).

PLEASE NOTE:  A hospital issued certificate of birth is not a legal document and generally is not accepted as proof of birth.

Mexican Matricula Consular and/or Mexican Electoral Cards, Foreign Birth Records, International Drivers License and Non-Citizen social security cards WILL NOT BE ACCEPTED AS IDENTIFICATION FOR BIRTH AND/OR DEATH RECORD REQUESTS
